Please note, home office evaluations are conducted virtually via Zoom. Workstation photos are required for all evaluations.

Steps and Services
Steps for UCSF Medical Staff Ergonomic Evaluations
- Complete the UCSF Health Ergonomics Training module in UC Learning.
- Review the photo instructions and attach them to the request form.
- Submit an ergonomic evaluation request form via this link.
- An evaluator will contact you to schedule an ergonomic evaluation. Methods may be via Zoom, phone call for troubleshooting or in-person.
Steps for UCSF Non-Computer Ergonomic Evaluation
Examples: Non-office areas (e.g., labs, clinical, custodial, radiology, food services)
Description: ergonomic assessments to address proper body mechanics, manual material handling tasks, and workspace design.
- Complete Cal Osha Ergonomic Training.
- Take pictures of action requiring evaluation.
- Submit an ergonomic evaluation request form outline the details of your request in the request.
- An evaluator will contact you to discuss the request and provide next steps.
Follow up Ergonomic Evaluation:
Already completed an evaluation but still experiencing issues.
Please submit a follow-up request through the link provided below so we can assist you further.
