Requesting an Ergonomic Evaluation

If you are a UCSF Campus employee, please visit the Campus Human Factors and Ergonomics team page for assistance or contact them at [email protected].  If unsure about which division you work for please check with your manager.  

Instructions for UCSF Health Employees 

  • Choose from the list of services below for the type of evaluation or consultation services needed. 
  • Please note, home office evauations are conducted virtually via Zoom.  Workstation photos are required for telework evaluations.
  • An evaluator will contact you within 2 weeks to either assist you virtually, by telephone triage or in-person.

Computer Workstation Evaluations

  1. Complete the UCSF Health Ergonomics Training module in UC Learning
  2. Review the photo instructions and attach them to the request form.
  3. Submit an ergonomic evaluation request form via this link. 
  4. An evaluator will contact you to schedule an ergonomic evaluation. Methods may be via Zoom, phone call for troubleshooting or in-person.

Non-Computer Workstations 

Non-office areas (e.g., labs, clinical, custodial, radiology, food services) ergonomic assessments to address proper body mechanics, manual material handling tasks, and workspace design.

Please submit an ergonomic evaluation request form and outline the details of your request.

An evaluator will contact you to discuss the request and provide guidance.
